Job Summary:
We are seeking a dedicated and compassionate Case Manager, registered ASW or LCSW to join our team.
Case Managers undertake a range of key responsibilities for clients that relate to healthcare, behavioral health, and housing.
Position will perform bio-psychosocial assessments and create care plans for clients and manage delivery of services with community partners.
This position will share time between our Clinic and our Medical Care Coordination Department.

Some of the Responsibilities Are:
· Provide in-person, community-based case management services that are strengths-based, client-centered and trauma-informed.
· Complete comprehensive biopsychosocial assessment to evaluate needs and create appropriate care plan.
· Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan.
· Provide crisis intervention for immediate housing needs, medical, mental health and substance use issues.
· Make recommendations and referrals to governmental or community-based programs, benefits and services.
Directly assist clients with linkage.
· Assist clients to access medical, mental health and substance use treatment providers, including accompaniment to appointments as needed.
· Work with hospital staff on appropriate discharge plan and follow up post discharge to assess and modify care plan as needed.
· Assist clients to function at most independent level possible by addressing ongoing psychosocial needs once clients are stabilized and not in crisis situations.
· Maintain strict confidentiality of patient information and ensure that patient privacy is maintained.
Qualifications:
· LCSW or MSW with ASW registration.
· Minimum two years’ experience working with vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions.
· Knowledge of principles, ethics and best practices of case management, care coordination, social work, and behavioral health services.
· Experience utilizing client-centered, trauma-informed practices to serve vulnerable populations including individuals experiencing homelessness, chronic medical conditions, and/or behavioral health concerns.
· Exposure to substance abuse disorders.
