Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use.
Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client’s specific needs and goals.
Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

Job Overview This is a union position.
The Pioneer Court Outpatient Program is committed to working with undeserved communities from a variety of backgrounds.
We strive to meet the ongoing needs of individuals struggling with mental health issues, substance use disorders, and dual diagnoses.
We happily accept clients from the San Mateo County ACCESS Call Center.
Counselors or Care Coordinators are an important part of the San Mateo County Outpatient Program.
They are responsible for assessing adult and youth client needs and strengths to collaboratively create treatment plans with their clients that support their health and recovery.
The care coordinator/counselor will provide individual and group counseling to adults and youth; deliver curricula in accordance with program-specific requirements.
Counselors assist clients in navigating systems of care while providing a supportive treatment environment.
Key Responsibilities Individual Treatment Responsibilities: Facilitates individual counseling sessions with each client on the caseload.
Offers clinical support and counseling to assist clients in meeting their treatment goals including providing feedback, setting boundaries, and celebrating successes.
Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status.
Treatment Setting Responsibilities: Facilitates group counseling sessions and teaches classes on various treatment-related topics.
Performs crisis intervention and communicates with the treatment team as unforeseen situations arise.
Attends required trainings and meetings for the agency.
Plans and facilitates client celebrations and special events.
May work weekends and holidays as needed.
Documentation Responsibilities: Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client.
Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner.
Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Targeted Case Management Services and other duties as assigned.
Education and Knowledge, Skills and Abilities Education and Experience Required: • Drug and Alcohol Certification recognized by DHCS and High School diploma or equivalent OR Bachelor's Degree in related field with Registration with Drug and Alcohol Certification recognized by DHCS (and become certified within 5 years or within the allotted time frame specified by DHCS, whichever comes first).
• First Aid Certified within 30 days of employment.
• CPR Certified within 30 days of employment.
Desired: • Bachelor's Degree in related field.
• Relevant experience working in outpatient settings.
• Application of Gender Responsive and Trauma Informed principles and interventions.
• Spanish speaking.
Background Clearance Required: • Ability to obtain and maintain background clearance with successful discharge from probation or parole, and absence of drug/alcohol related arrests or convictions, for at least 3 years.
Knowledge Required: • Culturally competent and able to work with a diverse population.
• Strong proficiency with Microsoft Office applications, specifically Word, Outlook, and Internet applications.
• Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
Skills and Abilities Required: • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.
