At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.
The Primary Care Lead Physician will support members experiencing homelessness at San Mateo County’s new Navigation Center, located at 275 Blomquist in Redwood City.
The Navigation Center provides safe temporary living spaces for individuals and couples along with intensive support services.
These services will be designed to help clients find and maintain stable housing.
The Primary Care Lead Physician will provide clinical leadership and oversight for Advanced-Practice Practitioners (NPs or PAs).
In addition, the Primary Care Lead Physician will perform direct patient care to a panel of patients.
Responsibilities include: Provide direct care to patients experiencing homelessness.
Triage, assess, screen, diagnose and treat patients either in-person or via telephone or video visits as needed.
Provide care at patient’s site of residence which may include the streets, tents, shelter, interim housing sites or mobile unit.
Review laboratory, radiologic or other diagnostic tests.
Prescribe medications.
Document in the electronic medical record.
Perform medical procedures as needed.
Collaborate with a multi-disciplinary team of social services and clinical personnel to develop a comprehensive care plan.
Supervise Advanced Practice Practitioners (APPs) including Physician Assistants and Nurse Practitioners in managing complex patients through direct field observations of team members, seeing members in tandem with the team, and providing remote guidance.
Be available to provide in-person, telephone or virtual assistance as needed during work hours.
Participate in Quality Care and Peer Review meetings.
Create schedules, approve time-off requests and arrange coverage for APPs as needed to ensure patient care needs are met.
Mentor, help educate, and provide feedback to APPs.
Participate in patient and in-service training for support personnel.
Coordinate with other primary care physicians, hospital personnel, specialists, and social services staff to coordinate care for post-acute care patients.
Participate in community stakeholder meetings to develop and maintain relationships with community-based organizations with mutual patient-care goals.
Other projects and duties that promote patient care and Healthcare in Action’s overall mission as assigned.
Knowledge of the treatment of complex chronic diseases, social determinants of health, psychiatric illnesses and substance use disorder diagnosis and management.
Flexibility to work in a start-up with changing goals/metrics.
Ability to identify team needs and coordinate with other resources/programs appropriately.
Knowledge of HCC and ICD-10 coding and documentation.
Knowledge of wound care and management.
Strong interpersonal skills and change agent mindset needed to build relationships with healthcare team and patients.
Cultural competence and sensitivity required, multilingual skills preferred, especially Spanish.
Knowledge of and ability to maintain HIPAA requirements.
Adaptable and open to change and problem-solving.
Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology Proof of Valid Driver’s License Maintain State Liability Insurance Have access to a Vehicle.
Qualifications and experience: MD or DO, Required Family Medicine, Internal Medicine, Emergency Medicine trained, Preferred Unrestricted California State Medical License, Required Board Certification in one of ABMS categories, Required 1+ years of related experience as a physician, preferably in a primary care, urgent or emergency care, or hospital setting Mentorship or direct people supervision strongly preferred.
