Homeless and disabled (san jose downtown)

I'm in the San Jose area, I have verifiable income.
Just got sick and stuck here with nowhere to go.
No shelter will accept me because I have limited mobility and no nursing facility will because of insurance.
I'm trying to find a place to work with me so I won't have to sleep in the streets.
